Navigating healthcare with hand-in-hand support.

We help you get the medical care, housing support, and life skills you need. If dealing with doctors, housing, or paperwork feels overwhelming, you don't have to do it alone. We walk with you, visit you in person, and stand by your side every step of the way. We treat you with the respect you deserve, and we will never leave you behind.

Our Difference

A Trauma-Informed Framework

We don't just route you to directories or mail paperwork. Our model bridges clinical rigor with direct personal companionship to remove transitional barriers completely.

Persistent Presence

Transition plans fail when communication breaks down. We maintain continuity of care by starting our connection inside regional holding spaces, continuing to walk with you directly through clinical lobbies and medical visits post-release.

Holistic Stabilization

Physical stabilization leads to lasting healing. By eliminating critical resource bottlenecks within the initial 72 hours—like medical cards, identification, and phones—we ensure you can focus safely on restorative clinical care.

Brittany Lee

Brittany Lee, MSW

Executive Director & Founder

Growing up in California, Brittany always felt a deep, calling pull to lift up those who had been overlooked by society. As a native of the Golden State, she has dedicated over 15 years of hands-on experience in social services to walking alongside unhoused adults and children, justice-involved individuals, foster youth, and individuals on probation—learning firsthand that true healing begins with genuine empathy, active companionship, and unwavering support.

Her clinical expertise is backed by a Master of Social Work (MSW) from San Jose State University, a Bachelor’s degree in Marriage and Family Studies, and associate degrees in Sociology and Administration of Justice. When she founded Community Connections of CA, Brittany’s vision was to create more than just a resource directory; she wanted to build a community family that walks hand-in-hand with participants every single step of the way, weaving clinical precision and deep personal compassion into everything the organization does.
